The subfeed breakers are mounted within the box but below the standard interior branch spaces. They do not use up the normal interior branch space.
Subfeed circuits do count toward circuits in the panel. So if you have a panel that must meet the 42 circuit rule then (2), 3 pole sub-feed breakers would count as part of the 42 circuits regardless if they take up space in the interior.
