The UL service entrance label is not available , nor required per NEC for Medium Voltage Equipment. The UL service entrance label is for voltages 600V and below.
SQ D MV Switchgear is UL listed , if a SE Label is required, SQ D/SE would send a nameplate to identify the switch as the service disconnect. This request would be directed to a Local SQ D/SE Sales office for quotation
