The replacement overload relay depends upon the device used and any Forms originally ordered. The following outlines the replacement parts:

Two pole starter with melting alloy (no overload form), 9065SDO11.
Three pole starter with melting alloy (no overload form), 9065SDO12.
Four pole starter with melting alloy (no overload form), 9065SDO13.
Five pole starter with melting alloy (no overload form), 9065SDO14.
Overload relay Form Y59, 9065SHA01Y59. *
Overload relay Form B, 9065SHA01Y59. *
Overload relay Form H10, 9065SR310. #
Overload relay Form H20, 9065SR320. #
Overload relay Form H30, 9065ST320.

* Bimetal overload relays are no longer available, please use either a melting alloy or solid state overload in it's place
# The basic motor logic overload relays (forms H10 and H20) are no longer available, please use the feature unit solid state overload relay in its place (form H30)
