How can one get a label to show a NQ or NF RTI panel is series rated?

A yellow and black label is included in the hardware bag that comes with the panel.
It can be filled out by the customer to note the tested series rating info in the digest or other recent spreadsheets . This label then can be affixed to the panel deadfront.
