Cause:
Wiring assistance

Resolution:
If wiring for 208V - 115V bring the 208V to H1 and H2 and the output to X1 and X2. Ground X2 per the NEC code to be the neutral.
If wiring for 230V - 115V bring the 230V to H1 and H3 and the output to X1 and X2. Ground X2 per the NEC code to be the neutral.
If wiring for 460V - 115V bring the 460V to H1 and H4 and the output to X1 and X2. Ground X2 per the NEC code to be the neutral.

When grounding per the NEC code, LAHJ will also need to be consulted as well.
