The replacement overload relay depends upon the device used and any Forms originally ordered. The following outlines the replacement parts:

Two pole starter with melting alloy (no overload form), 9065SDO4.
Three pole starter with melting alloy (no overload form), 9065SDO5.
Overload relay Form H10, 9065SFC20 Class 10/20
Overload relay Form H20, 9065SFC20. Class 10/20
Overload relay Form H30, 9065SFC20.
Overload relay Form H308, 9065SFB20.
Note: Starters with ``H`` Forms cannot be used on single phase applications.
