When ordering a factory modification form for the 8941 Duplex Controller, does it provide the option for both starters?

Do the factory modification forms for the 8941 Duplex Controllers apply for both starters?

Resolution:
Only if the forms are the ones listed on page 16-96 of Digest 177. Any other forms will require the use of the form letter twice.
For example; to obtain hand-off-auto (Form C) use the form as C but if the modification is for a green off pilot light the P2 form would be required to appear twice like P2P2.
