Resolution:
Use a QBL32100 with a NQMB2Q or HDL36100 or JDL26150 with a NQMBHJ or similar 3 pole breaker per the * footnote on p. 9-7 of the digest 176. The B phase lug on the breaker can be removed to avoid confusion. The bus links on the outer poles of the adapter kit and the deadfront cutout only works with 3p breakers.
