Which handle and shaft do we use with the old LK3QU3 400A switches?

Use the GS2AE61 (15.7" shaft) and GS2AH150 for black handle and GS2AH160 for red/yellow handle. Will also need the GS2AH100TO200 adapter for the handle. Both handles are rated Nema 4/4X
Both handle and shaft needs to be replaced.
