Cause:
Ground fault circuit breakers can be used on solidly-grounded systems, but what about resistance-grounded systems.

Resolution:
Ground fault circuit breakers are for use on solidly-grounded systems only. If the system is high resistance to ground (or an ungrounded system), the amount of ground fault current may not be sufficient to trip a ground fault breaker.
