Masterpact NW is not communicating breaker status correctly

The cause for this is usually incorrect installation of the Communication Switch Assembly.

First ensure that the silver colored shaft coming down vertically from the SDE switches is free to move up and down. If not, grasp the shaft with a needle-nose plier and move it up and down until it is free.
If this doesn't correct the issue, remove and re-install it per the linked instruction bulletin 48049-139-02.

If the switch assembly is broken or is still non-functional, replace it. It is only available as a component of kit # S48384.
